Our highly experienced & qualified registered nurses conduct a full health assessment in conjunction with the participants and their families to develop an individualised holistic care plan.
Similarly, our team of experienced disability support workers/carers are always ready to offer participant tailored services in line with each participant’s goal of care and NDIS needs.
Also, CLCS provides supports for a whole range of people with
acute and chronic medical health conditions, people living with disability (self-funded, plan managed & NDIS funded), enduring mental illness and elderly people on commonwealth funded aged care package.
Meet the Team
CLCS has nursing staff with over 20 years experience serving the Territory. A dream birthed by persons whose sole desire is to help bridge the medical gap between the consumers and healthcare providers.
Also, CLCS has a team of committed disability support workers (DSWs), well trained and equipped to provide holistic participant-centred care to our participants with different funding such as private funding, self-plan managed, or NDIS funded in supported living accommodation (SIL).
