Disability Services

CLCS is an NDIS organisation awaiting full registration. However, we are fully open and accepting referrals for self-managed and plan managed clients.

 

As a newly birthed community nursing and disability services, CLCS is out to bring expertise, commitment, dedication, honesty, transparency, truth and sincerity into the disability sector.

 

Like our motto says, ‘Client-centred care, we are poised now than ever before to offer our participants support services tailored to their needs and set out goals. CLCS will assist each of our participants meet and exceed their set goals.

Supported Independent Living (SIL)

  • CLCS operates a range of supported independent living accommodation services which encourages choice, control and community inclusion for persons living with disability.

  • CLCS also provides accommodation for individuals with Intellectual, complex & forensic mental health needs.

Accommodation

  • ​Short term and medium-term accommodation for an agreed limited period.

  • Respite accommodation with supported independent living.

  • Supported independent living accommodation in fully furnished housing with support services available from a few hours to 24/7.

Assistance with Personal Activities

CLCS strive to assist you with daily activities and routines to help you continue living freely and independently as well as paying attention to your wellbeing and health.


We assist with:
- Personal hygiene and grooming
- Grocery shopping
- Appointments
- Cooking
- Laundry

Social and Community Participation

It can be challenging to stay busy and active when living with a disability. CLCS organise and facilitates programs and activities for the participants to help promote resilience, personal growth, develop friends, skill building and assist in social and community participation.

 

We help with the following:

  • Attending local events such as music festival

  • Participating in activities such as bowling, art and craft and bingo

  • Support for skill development

  • CLCS will assist our participants access reliable transport services while also providing the needed personal support either as an individual or as a group.

Assist Life Stages: Part 1

At CLCS, we are committed to helping you get the most out of your NDIS support plan and achieve your goals. Assist life stages support includes: 


Level 1: Support Connection
Support Connection is time-limited assistance provided to participants to strengthen their ability to connect with informal mainstream, and funded supports, and to improve their capacity to maintain support relationships, settle service delivery issues, and participate in the NDIS processes. We help participants in:

 

  • Identifying/investigating options

  • Understanding the flexibility of funds

  • Reaching decisions regarding services

  • Reaching an agreement with providers

  • Starting service and ensuring new support arrangements thrive

Assist Life Stages: Part 2

Level 2: Coordination of Supports
Coordination of Supports is designed to strengthen participants’ ability to improve their capacity to maintain relationships, manage service delivery tasks, live actively & independently, and get engaged with the community.

 

This support is focused on helping participants to build the skills they need to understand and implement their support plan. We are here to help you with:

  • Capacity building skills

  • Independent self-care and living skills

  • Crisis and conflict resolution

  • Financial mentoring

  • Daily planning and budgeting

  • Create supportive networks within the community

  • Connect to other supportive community services and programs

  • Life planning, development and goal planning

  • Assistance with accommodation and tenancy obligations

  • Life transition planning including mentoring, peer support and individual skill development

  • Assistance with decision making

Transportation & Travel

We provide transport for our participants for their various activities or appointments.


Transport is available for:

  • Attending school or work

  • Medical appointments

  • Family meetings, dinners etc


CLCS is currently accepting referrals for self-funded and plan managed clients.
