In-Home Diabetes Care Services

Living with diabetes is a challenge on its own, considering the lifestyle changes, dietary changes, blood sugar and insulin management.

 

CLCS is here to help lessen the challenges associated with living with diabetes or to help the participant transition into the new lifestyle changes. Care plans are drawn up for participants by registered nurses together with the participant or the family to help manage the blood sugar levels and medication administration.

 

Meals are prepared by the support workers for the participant in line with an agreed meal plan set by the multidisciplinary team (doctor, dietician, or nutritionist) such as low salt, low cholesterol, low fat, and less carbohydrates.

Diabetes Care Plan

Your Diabetic Care Plans Includes:
 

  • Proper Diet and regular exercise plan

  • Compliance to prescribed medications

  • Attending all Scheduled appointments with allied health professionals (podiatrists, dietician, & diabetes educators).

  • Following your Individualised diabetic health goals.

  • Attending Regular care plan appointments. This gives the participant the opportunity to set realistic goals, relieve experiences, and discuss any worries.


In CLCS, we know that healthy eating is the bedrock of healthy living, with or without diabetes. But if you have diabetes, you need to know how food affect your blood sugar levels. It’s not only the type of food you eat but also how much you eat and the combinations of food types you eat.
