Mental Health Outreach Mobile Clinic / Emergency Homelessness & Shelter Support Services

CLCS runs a specialised outreach mobile nurse-led depot clinic administration services.

 

The clinic is an enhanced primary care in mental health, with a focus on relapse prevention, rehabilitation and quality of life.

 

The clinic supports with long acting injectable (LAIs) administration-which has the ability to slowly release desired amount stretched over an extended period of time.

 

We welcome referrals from In-patient and community public mental health services, private psychiatrist services, GPs, and NGOs.

Primary target population who may benefit from our outreach nurse-led depot clinic are clients:

  1. From remote, on respite in Darwin

  2. With failed GP discharge

  3. Staying in step up/down accommodations

  4. Staying at 24/7 supported accommodations

  5. Requiring home visits

  6. Requiring 2-hours post monitoring

  7. Requiring After hours depot support

  8. Not keen on GP discharge

  9. Not keen on continuing depot with public MHS

  10. With no case management needs except depot

  11. IPU discharge, on depot with no case manager allocation needs

  12. Interstate visitors requiring depot support whilst visiting

Emergency support shelter

  1. 24/7 emergency shelter for up to 2 weeks.

  2. 24/7 step up step down support up to 8 weeks

  3. Meal support

  4. Medication supervision

Homelessness support program

  1. 24/7 support for up to 12 weeks

  2. Meal support

  3. Medication support
